CDC Reservations

This reservation request form is intended for CDC reservations for dog imports arriving via IAD-Washington-Dulles International Airport ONLY. Please allow 3-4 business days for review of your request and documentation. Please note that incomplete documents submission will delay the reservation confirmation process, so please upload all necessary documents at the time of reservation request (with the exception of documents only available immediately prior to travel).

IMPORTANT: A request does not constitute a reservation. A reservation is not complete without written reservation confirmation from our facility and the CDC.
