CDC Reservations

This reservation request form is intended for CDC reservations for dog imports arriving via IAD-Washington-Dulles International Airport ONLY. Please allow 3-5 business days for review of your request and documentation. Please note that incomplete documents submission will delay the reservation confirmation process, so please upload all necessary documents at the time of reservation request (with the exception of documents only available immediately prior to travel). Please do not submit multiple request forms, as this will slow the reservation process. If you submit one and your arrival date changes, please e-mail us at cdcreservations@penderpetretreat.com. IMPORTANT: A request does not constitute a reservation. A reservation is not complete without written reservation confirmation from our facility and the CDC.

*NOTE: We recommend starting the reservation process at least 30 days in advance. Additionally, we recommend waiting to book your flights until we have confirmed availability on the date you are requesting for arrival to avoid airline cancellation or change fees whenever possible.*
