This reservation request form is intended for CDC reservations for dog imports arriving via IAD-Washington-Dulles International Airport ONLY. Please allow 2-3 business days for review of your request and documentation. A request does not constitute a reservation. A reservation is not complete without written reservation confirmation from our facility and the CDC.
